Financial Policy

Medicare:

As participating providers with Medicare, we file assigned claims directly to Medicare. The patient is responsible for the 20% co-payment not paid by Medicare and the $135 calendar year deductible. Secondary insurance are filed as a courtesy to the patient. Since many out of state insurance companies either do not cover office visits or pay the insured party directly, the 20% co-payment for office visits will be collected at the time of service.

Manage Care (HMO, PPO):

We participate with many managed care companies. Prior authorization may be required by your insurance company. Please contact your primary physician and/or insurance company regarding approval for your appointments; otherwise, your insurance company may put the responsibility of payment on you, the patient. Applicable co-payments for office visits will be collected at the time of service. Please call our office if you have any questions regarding insurance participation.

Medicaid or Social Service:

We will file insurance, however, we need proper authorization for treatment and documentation of eligibility.

Other Plans:

Our policy is to collect payment for office visits at the time of service. For your convenience we accept VISA, Master Card, American Express and Discover. We will be happy to file insurance for your reimbursement.
